MFGP Cancellation Form


We hate to see you go! Please fill out this form and we will be in touch with you shortly to finalize canceling your membership.


Cancellation Policy: Members may cancel their membership at any time with a 14-day written notice. All cancellation requests must be submitted exclusively through this form. Requests made through email, phone, or in person will not be accepted.


If a scheduled membership payment falls within this 14-day window, that payment will be charged in full, regardless of attendance or usage during the notice period and the member will retain access to the facility and classes through the end of their final billing cycle.


All membership payments, including the final payment, are non-refundable.
